Ventricular Assist Devices in Modern Heart Failure Care: Clinical Advances and Mechanistic Foundations

Abstract
Ventricular assist devices (VADs) have evolved from bulky pulsatile systems to durable continuous-flow pumps that provide lifesaving therapy for patients with advanced heart failure. Contemporary devices, particularly left ventricular assist devices, are now established in clinical practice as both bridge-to-transplant and destination therapy, with survival rates approaching those of heart transplantation. Despite these advances, complications such as pump thrombosis, drive-line infections, bleeding, and neurological events remain major limitations and significantly impact patient outcomes and healthcare costs. Basic research continues to focus on improving bio-compatibility, minimizing hemocompatibility-related adverse events, and developing fully implantable systems that can reduce infection risks. Emerging technologies, including wireless energy transfer, machine learning–based patient monitoring, and biohybrid device surfaces, promise to transform mechanical circulatory support by enhancing safety and promoting myocardial recovery. Pediatric applications and cost-effectiveness remain under-explored but represent critical dimensions for future adoption and equitable access. This review highlights the progress in clinical application and basic research of VADs, highlights key complications and management strategies, and discusses the future directions that may enable broader integration of VADs as a standard of care in advanced heart failure.
